29 CFR 1926.652(a)(1):Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a). On or about August 4, 2022 3 employees were laying/installing new 10" sewer/drain pipes in a new parking area of a shopping center when the excavator broke a 12" water supply line hidden underground and not on any existing utility location plans. While attempting to excavate and locate the pipe 2 men were working in the 12 foot deep excavation with no means of cave in protection.
