29 CFR 1910.29(b)(2)(i):The employer did not ensure that midrails were installed at a height midway between the top edge of the guardrail and the walking working surface: (a)Gage Brothers Concrete Products, Inc. at 2810 North Bahnson Avenue, Sioux Falls, SD 57104: On or about and at times prior to July 28, 2022, the employer did not ensure a midrail was installed between the center walking-working surface and the top handrail of the seven A-frame cement slab holding structures located on the Production Floor and in the Finish Products areas. This condition exposed employees to falls hazards greater than four feet.
