29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about July 26, 2022, at jobsite located at 222 Meigs St., Sandusky, OH 44870, Underground Utilities Inc. did not ensure a safe means of egress, such as a ladder was placed in the 7 feet and deeper trench in which an employee was working. One employee was exposed to cave-in hazards and injury, which can occur from trench cave ins, and the inability to exit the trench quickly in case of an emergency.
