29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. At a worksite located at 800 Atlantic Street, Kansas City, Missouri. Employees were exposed to serious slip, trip and fall hazards created by the large hole in the production floor leading to and from the loading dock, plywood was put in the hole so the Yale Forklift serial # A390V12530T could travel through the accumulated water.
