29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) For equipment at the establishment such as, but not limited to, laminators, pleaters and glue machines, on or about 07/21/2022 and at times prior to, the employer exposed employees to potential amputation and electrical shock hazards, in that, lockout tagout procedures were not available and being used for maintenance/servicing work on this equipment. Hazardous energy existed in the form of 220 VAC, compressed air, and mechanical moving parts.
