29 CFR 1926.404(f)(6):Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On or before July 20, 2022, at a worksite located at 134 Chapman Drive, Lot 324, Pataskala, Ohio 43062: The employer failed to ensure employees used electrical equipment where the path to ground was permanent and continuous. Employees used an extension cord that was missing the ground prong, thereby being exposed to a 123-volt electrical contact hazard.
