29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) Main Pump House: On or about July 14, 2022, and at times prior, Employees were exposed to fall hazards while working from the roof near an opening approximately 2 feet wide by 6 feet long at a height of approximately 9 feet from the ground below during pump install and removal.
