29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On or about July 13, 2022, at an establishment located at 1101 Perry Street in Algoma, Wisconsin; the employer did not ensure that only stable or safely arranged loads were handled when an employee used a CAT CC25K (S/N: AT82C-01372forklift to adjust a wear panel on a door mounted on a hammermill encasement. The door came off the hinge when the vehicle's fork made contact with the bottom of the door, causing the door to fall and expose nearby employees to crushed-by hazards.
