29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about July 12, 2022, and at times prior thereto, employees did not receive training on hazards associated with working in and around an excavation up to approximately 6 feet deep, exposing employees to hazards such as, but not limited to being struck-by materials and equipment and cave-in.
