29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components were operationally intact: At Stratas Foods LLC, on or about July 1, 2022, Austin Industries, Inc. exposed an employee to thermal burn hazards from the 2 inch Canola Oil Byproduct Line downstream of Tank 70 that had a 1/4 inch ball valve that did not have a working safety slide mechanism and end cap or pipe plug. Before lockout or tagout devices were removed the employer did not inspect the work area to ensure that equipment components were operationally intact.
