On or about July 7, 2022, at the address of 9773 State Route 224, Findlay OH 45840: The employer did not ensure employees were protected from falls while performing roofing work activities atop a single-story residential structure with multiple roof pitches. Employees operating in this conditions were exposed to falls of 10'- 4" or greater to the ground below. Vega Renovations LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1524251, citation number 1, item number 4 and was affirmed as a final order on August 5, 2021, with respect to a workplace located at 6117 Alexa Ln., Sylvania, OH 43560 Vega Renovations LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1506779, citation number 1, item number 2 and was affirmed as a final order on April 6, 2021, with respect to a workplace located at 2024 Collingwood, Toledo OH 43608
