29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were conducted by a competent person: a)Oftedal Construction, Inc. at 10755 Sheridan Lake Road, Rapid City, SD 57702: On or about and at times prior to June 29, 2022 the employer did not ensure employees in the vicinity of a Caterpillar 14H Motor Grader, Model Number 1411, were protected from struck-by hazards. On June 29, 2022, an employee walking across the work zone was fatally injured after being struck-by a motor grader driving in reverse.
