29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Spangler Candy Company at 400 N. Portland Street, Bryan, Ohio 43506: On or about June 10, 2022: The employer failed to ensure energy control applications steps to control the electrical and pneumatic energy sources on the Kneader #4 machine were implemented prior to employee(s) performance of servicing activities on the machine's proximity switch. Employees performing these work task in an energized condition were exposed to caught in hazards at the machine's rotating table. Spangler Candy Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d) which was contained in OSHA inspection number 1270992, citation number 2, item number 1 and was affirmed as a final order on April 11, 2018. with respect to a workplace located at 400 N. Portland Street, Bryan, Ohio, 43506.
