29 CFR 1926.959(d)(1): Mechanical equipment was operated so that the minimum approach distances established under 1926.960(c)(1)(I) were not maintained from exposed energized lines and equipment: On or about June 15, 2022, at an electrical sub-station, an employee in an aerial man lift approached an energized line within the established approach distance, exposing the employee to electrical hazards.
