29 CFR 1926.501(b)(4)(ii): The employer failed to protect employees and site workers from tripping in or stepping into or through holes (including skylights) by covers: a) Jobsite located at 2508 E Cumberland St, Philadelphia, PA: The employer failed to protect employees and site contractors performing work at or near an unguarded 3 foot by 7.75 foot floor hole on the first floor in that employees removed but did not secure the floor hole cover after use, potentially exposing employees and site contractors to fall hazards of approx. 12 feet above the next lower level. Condition noted on or about June 13, 2022. JVL Company Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1295557, citation number 2, item number 1-a and was affirmed as a final order on 7-23-2018, with respect to a workplace located at 601 Christian Street, Philadelphia, PA 19147. JVL Company Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1069282, citation number 1, item number 2 and was affirmed as a final order on 12-18-2015, with respect to a workplace located at 1735 and 1743 East Moyansing Avenue, Philadelphia, PA 19148.
