29 CFR 1904.41(a)(1): Annual electronic submission of part 1904 records by establishments with 250 or more employees. The establishment had 250 or more employees at any time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form: On or about 6/8/2022 and times prior thereto, at the Steward North Shore Medical Center Inc. located at 1100 NW 95th Street, Miami, FL 33150, it was determined that the employer failed to electronically submit information from their OSHA Form 300A or equivalent for the 2021 calendar year by the established deadline of March 2, 2022. The establishment employed 876 employees and is classified as 622110 - General Medical and Surgical Hospital during calendar year 2021.
