29 CFR 1910.184(c)(6):Slings were not securely attached to their loads: A) Elyria Foundry LLC, on or about June 3, 2022, at the address of 745 Bullocks PKWY Lorain Ohio: The employer did not ensure that lifting devices were securely attached to the load. The hooks of the 2-legged chain sling were not fastened to the castings to prevent displacement. Employees performing lifting operations in this condition were exposed to struck-by hazards.
