29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. McClean Affiliates, located at 400 North Main Street, Bristol, CT 06010: The employer did not record employee work-related COVID-19 infections on the OSHA 300 log or its equivalent on the following occasions: a) A healthcare worker tested positive for COVID-19 on June 2, 2022, when multiple residents were confirmed COVID-19 positive. Worker had a minimum of five lost workdays. b) A healthcare worker tested positive for COVID-19 on June 5, 2022, when multiple residents were confirmed COVID-19 positive. Worker had a minimum of four lost workdays. Abatement Certification and Documentation is required
