Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1926.416(a)(1): Employees were permitted to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: (a) On or before June 3, 2022, the employer did not ensure that electrical circuits were de-energized prior to employees removing fluorescent light fixtures in the ceiling of a commercial building and then installing new LED light fixtures that operated on 277-volts. An employee who was working from a six-foot A-frame ladder to install a new LED light fixture was found unresponsive on the floor by another employee. The employee was hospitalized with serious injuries and later died on 6/10/22. The electrical circuit was not de-energized. This condition exposed employees to electrical shock hazards. (b) On or before June 3, 2022, the employer did not ensure that electrical circuits were de-energized prior to employees removing fluorescent light fixtures in the ceiling of a commercial building and then installing new LED light fixtures that operated on 277-volts. Three employees performed the work and did not lock the circuit breakers in the off position. This condition exposed employees to electrical shock hazards
