29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. A) On or around June 2, 2022, the employer did not ensure that at least two employees certified to operate forklifts were reevaluated every three years. Tripack LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1370163, citation number 1, item number 1B, and was affirmed as a final order on February 19, 2019, with respect to a workplace located at 12025 Tricon Rd, Cincinnati, Ohio 45246.
