29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. In the Pipe Machine Area, at 6390 Philips Hwy in Jacksonville Florida. On about May 26, 2022, and at times prior: On the #1 Round Pipe Roller Machine, the employer exposed an employee to an amputation injury hazard in that the in-going nip point of the discharge rollers were not guarded when the machine was reversed to clear jams.
