29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: a) At or near 2183 Lemoine Ave, Fort Lee NJ: Employees were exposed to amputation hazards while operating winch cables on a garbage truck where the operation had not been assessed and a written certification of the assessment had not been performed, on or about 1/3/22. A worker had his index finger amputated while operating the cable on 1/3/22.
