29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest systems were not designed, installed and used under the supervision of a qualified person: On or about May 19, 2022, on the 2:12 pitch roof of the industrial building located at 8351 NW 93rd Street, Medley, Florida, during a commercial roofing project, the horizontal lifeline was not capable of supporting at least 5,000 pounds per employee, exposing employees to a 34-foot fall hazard.
