29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling: On or about May 13, 2022, the employer at 2421 Morral Kirkpatrick Road in Morral, Ohio failed to install effective fall protection for employees in the wash bay area when employees first get on top of the tanker trucks approximately 10 feet or higher and when they serve as hole watch for employees cleaning the inside of the tanker truck. Falls of 10 feet or greater can cause serious injuries and possibly death if an employee falls off the side of the truck.
