Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. (a) On or about May 06, 2022, for the employees who were exposed to struck-by hazards where the BVM elevator was not fully latched while tripping 3 1/2 inch pipe out of the hole at well site identified as HESS - AN-DINWOODIE - 153-94-2833H-5, NWNW 28-153N-94W: Near 44 Street NW & 98 Avenue NW, New Town, ND. Among other measures, one feasible and acceptable method to correct this hazard would be to follow BVM Series "TA" Elevator Maintenance Manual - Operation pg. 3. Which states. "Prior to hoisting with the elevator from any position, it is necessary to ensure that the elevator is completely installed around the pipe and that the latches are properly engaged. Failure to do this could result in serious injury."
