29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about 06/28/2022 at 3856 Flatlands Avenue, Brooklyn, NY 11234, technicians applying hazardous chemicals such as but not limited to 0.5-1.5% quartz during the production of prosthetics wore half-face tight fitting 3M respirators under voluntary conditions. The employees were not provided the respirator information in Appendix D. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
