29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: Worksite On May 4, 2022, three NorthStar employees were injured when a concrete mezzanine platform on the west wall of building 1A, unexpectedly collapsed during demolition and asbestos abatement operations. On or about May 4, 2022 employees were exposed to hazards such as mezzanine floor collapse, struck-by and crushing hazards while working where employer had not trained employees to recognize and avoid the hazards associated with cutting wooden posts.
