29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided and used: (a) 8B Hydrogen System: On April 26, 2022 and times prior, employees were not provided or required to wear flame retardant clothing (FRC) while working within the hydrogen system which was had an electrical classification of Class I Division 2. Employees were exposed to potential flash fire hazards while working around tanks, piping and equipment which contained flammable hydrogen gas.
