29 CFR 1926.451(d)(5)(ii):Suspension scaffold support devices such as cornice hooks, roof hooks, roof irons, parapet clamps, or similar devices were not supported by bearing blocks. Instance A) On or about April 25, 2022 at 118-37 Metropolitan Avenue, Kew Gardens, NY 11415 Parapet hooks/cornice hooks used to support a two (2) point suspended scaffold being used by employees while performing exterior brick work on the 3rd and 4th floors of a residential apartment building were not supported by bearing blocks. Instance B) On or about April 26, 2022 at 118-37 Metropolitan Avenue, Kew Gardens, NY 11415 Parapet hooks/cornice hooks used to support a two (2) point suspended scaffold accessed by an employee on April 26, 2022 was not supported by bearing blocks.
