29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) On or prior to April 21, 2022; the employer did not instruct employees in trenching and excavation hazards including potential cave-in hazards prior to working in trenches and excavations. Employees were working to install two 6" conduits in a trench without cave-in protection. The trench measured approximately 4.0-6.0 feet deep, had nearly vertical walls and was Type B soil next to a previously installed utility line.
