29 CFR 1910.23(c)(10): The employer did not ensure that the top of a non-self-supporting ladder is placed so that both side rails are supported, unless the ladder is equipped with a single support attachment: On or about 04/18/2022, at a worksite located at 200 Jungle Road, Palm Beach, Florida, an employee was exposed to fall hazards when the top of an extension ladder being used was not secured to the irregular surface of a palm tree.
