29 CFR 1910.140(c)(20): The employer did not ensure that ropes, belts, lanyards, lifelines, and harnesses used for personal fall protection are protected from being cut, abraded, melted, or otherwise damaged. a) Hansen Mueller Company on or about April 14, 2022, at the address of 1800 N. Water St. Toledo, OH: The employer did not ensure that the personal fall arrest system, located at the rail loading conveyor platform was protected from components of the personal fall arrest system being damaged. Damages to include abrasions to steel braided lifelines that catch on sharp/rough surfaces of conveyor housing, up to catastrophic failure of the fall arrest system due to the lifeline component being completely torn from the connecting device anchored above when contacting unguarded rotating shaft of conveyor. In this condition employees were exposed to hazards such as but not limited to falls over 15ft. to the stone surface below.
