29 CFR 1910.147(f)(1)(v): Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance. a) On April 4, 2022, in the warehouse, an employee was conducting servicing and maintenance on the Bobst, Flat Die Cutter Machine, model SPO 1600, while testing the newly installed switch, the machine cycled while the employee was inside the machine. The employer did not ensure that employees testing the equipment deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue servicing the equipment.
