29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Rumpke Waste & Recycling Services located at 431 Prospect Dr. Findlay OH 45840: On or about April 11, 2022, the employer did not ensure specific energy control procedures were utilized for the control of hazardous energy while performing maintenance and service activities on the hydraulic lifting arm(s) of a front load refuse truck. Employees operating in proximity of the loading arms were exposed to struck by, caught in, and crushing injuries from hydraulic and gravitational energy sources while engaged in the aforementioned activities.
