Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) 1325 10th Avenue South, Great Falls MT, Bay 1: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Model #: XLA to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were free of damaged and defective components in that the automotive lift was being operated with damaged loading ramps. b) 1325 10th Avenue South, Great Falls MT, Bay 1: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Model #: XLA to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were free of damaged and defective components in that the South most lift jack of the automotive lift was missing a seal. c) 1325 10th Avenue South, Great Falls MT, Bay 1: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Model #: XLA to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were placed on the lock while employees worked beneath vehicles on the automotive lift. d) 1325 10th Avenue South, Great Falls MT, Bay 1: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Model #: XLA to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were free of damaged and defective components in that the automotive lift was being operated with a missing bolt connecting the vehicle lift runway to the floor post. e) 1325 10th Avenue South, Great Falls MT, Bay 1: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Model #: XLA to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were free of damaged and defective components in that the automotive lift was being operated with a damaged runway to post connection point. f) 1325 10th Avenue South, Great Falls MT, Bay 4: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Serial # 282922: to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were free of damaged and defective components in that the automotive lift was being operated with a leaking hydraulic floor post seal. g) 1325 10th Avenue South, Great Falls MT, Bay 4: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Serial # 282922: to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were free of damaged and defective components in that the automotive lift was being operated with a gouge in the floor post damaging the floor post seal. h) 1325 10th Avenue South, Great Falls MT, Bay 4: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while utilizing the damaged Western automotive lift Serial # 282922: to conduct vehicle maintenance. The employer failed to ensure that the automotive lifts were placed on the lock while employees worked beneath vehicles on the automotive lift. i) 1325 10th Avenue South, Great Falls MT, Bay 1: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while opening and closing the Clopay overhead garage door that contained a stem roller outside of the overhead track. j) 1325 10th Avenue South, Great Falls MT, Bay 4: On or about April 4, 2022 and at times prior, employees were exposed to struck by hazards while opening and closing the Clopay overhead garage door that contained a stem roller outside of the overhead track.
