29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) On or about April 4, 2022, and at times prior, at the residential construction site located at 4401 Elk Ridge Drive in Bismarck, North Dakota, the employer did not inspect all components of the fall arrest system that was on site. The horizontal life line rope provided was frayed, damaged, and unserviceable.
