29 CFR 1926.651(h)(1):Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation.(a) On or about April 4, 2022, employees were exposed to a trench cave in while performing fire hydrant replacement located at 2770 Boston Road, Brunswick, Ohio 44212. The employees were working in a trench that was approximately 6 feet 5 inches deep without any form of cave in protection and the employer did not ensure that precautions were taken to remove the accumulated water.
