29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: (a) On or about March 28, 2022, at the worksite located at 1777 Wisconsin Ave, Grafton, WI, the employer did not ensure compressed helium gas was used in accordance with the Compressed Gas Association Pamphlet P-1-1965, "Safe Handling of Compressed Gases, section 3.2.6, where cylinders of helium were transported throughout the facility on a hand truck without an adequate means of securement, thereby exposing workers to helium related injuries and illnesses. (b) On or about March 28, 2022, at the worksite located at 1777 Wisconsin Ave, Grafton, WI, the employer did not ensure compressed helium gas was used in accordance with the Compressed Gas Association Pamphlet P-1-1965, "Safe Handling of Compressed Gases, section 3.4.4, where at least one tank of helium located in the front of the store was not adequately secured, thereby exposing workers to helium related injuries and illnesses. (c) On or about March 28, 2022, at the worksite located at 1777 Wisconsin Ave, Grafton, WI, the employer did not ensure compressed helium gas was used in accordance with the Compressed Gas Association Pamphlet P-1-1965, Section 3.2.1, "Safe Handling of Compressed Gases", section 3.2.1, where unused cylinders of helium where not provided with a cap when not in use, thereby exposing workers to helium related injuries and illnesses. Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.101(b), which was contained in OSHA inspection number 1529043, citation number 02, item number 01 and was affirmed as a final order on January 12, 2022, with respect to a workplace located at 1777 Wisconsin Ave, Grafton, WI 53024. Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.101(b), which was contained in OSHA inspection number 1501569, citation number 03, item number 01 and was affirmed as a final order on May 3, 2021, with respect to a workplace located at 1707 East Houston St., Cleveland, TX 77327. Dollar Tree Stores, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.101(b), which was contained in OSHA inspection number 1206871, citation number 1 item number 1 and was affirmed as a final order on July 11, 2017, with respect to a workplace located at 727 N Charles St., Warrensburg, MO 64093.
