29 CFR 1910.23(b)(1):29 CFR 1910.23(b)(1): The employer did not ensure that ladder rungs, steps, and cleats are parallel, level, and uniformly spaced when the ladder is in position for use: On or about March 24, 2022 at 200 West Railroad Street, Columbiana, OH 44408, the employer failed to ensure employees utilizing 12 foot step ladders to work on elevated equipment was safe in that the cleats were missing on several ladders.
