29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment, or components thereof: a) Automatic Re-Hanger Machine; On or about March 10, 2022, an employee was exposed to caught-in and crush-by hazard in that the employer allowed an employee to troubleshoot an operating conveyor system without being safely positioned.
