OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to struck-by/crushed-by hazards when hydraulic outriggers were not positioned to prevent vehicle rollover while overhead conveying equipment was positioned. a. On or about March 9, 2022, located at 3810 Slate Dr., Champaign, IL, the employer did not ensure all outriggers on a Putzmeister Belt Conveyor were locked into the working position in order to prevent a tip-over incident, thereby exposing an employee to struck-by and/or caught-between hazards. On March 9, 2022, a Putzmeister Telescopic Belt Conveyor tipped over onto an operator resulting in serious injuries.
