OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to crushed-by and/or caught-in hazards: On or about March 17, 2022, at the back of the facility, employees utilize the compactor to compact refuse such as, but not limited to cardboard. The safety interlock or sensor on the door into the compactor was not maintained in a working condition. Among other methods, one feasible and acceptable means of abatement would be to: (1) Ensure that the access door of loading chambers has an interlock system that prevents cycling motion while the access door is open. (2) Ensure that there is no damage to the interlock that prevents it from operating as intended. (3) Ensure that the wiring connections have not been damaged or altered to prevent operation of the interlock as intended. (4) Ensure that the use of tools or other materials are not used to intentionally defeat the interlock.
