29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. a) At the facility, U.S. Department of Veterans Affairs / Northport VA Medical Center: The employer provided and required employees to wear N95 respirators to protect them when exposed to patients suspected or confirmed positive for COVID-19. The employer failed to provide an annual fit test for employees; on or about 2/28/2022 - 3/3/2022. Notice #2, Item #1, 29 CFR 1910.134(f)(2) as required by 29 CFR 1960.8(b): The employer failed to provide an annual fit test for employees. The employer was cited for a violation of the similar standard on 3/7/2022, Inspection# 1552223, 4/4/2022 of final order/settlement, and means of abatement. Notice #1, Item #1, 29 CFR 1910.134(f)(2) as required by 29 CFR 1960.8(b): The employer failed to provide an annual fit test for employees.
