29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about February 22, 2022 employees were exposed to machine hazards associated with moving parts, in-going nip points, and struck-by hazards when plugging vent holes in the cope tooling for quality control purposes on Core Making machine 619. The employer failed to document that employees were trained on all the energy sources and the methods to isolate those energy sources.
