29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level: (a) Williams-Sonoma Stores, Inc. at 5503 Havana Street in Denver, Colorado: On or about February 24, 2022, and at times prior, employees were not protected from contacting a lower level when working at heights on a Raymond Order Picker 560-OP30TT. The personal fall arrest system did not provide adequate fall protection for the expected amount of clearance from the operator position to the lower level. This exposed employees to a fall hazard.
