29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about February 15th, 2021, employees were exposed to burn and caught-in-between hazards at Rotovac 7. In that, the buttons that operate the machine were not properly covered to prevent an employee from activating the machine while their hands were inside.
