Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: On or about February 16, 2022 and prior, at an establishment located at 900 Thilmany Road, Kaukauna, Wisconsin; at Paper Machine 14 on the Dry End; the Shaw Box 0006150 overhead overhung crane utilizing the Shawbox DMR-3, S/N: 58-37177 Hoist #23A (North) was defective and not taken out of service. Defects included but are not limited to: the hoist brake continued to drift down after the operator released the controls exposing employees to stuck-by hazards. The employer did not remove or tag the hoist out of service until repaired.
