29 CFR 1910.132(f)(1)(ii): The employer did not provide training to each employee who is required by this section to use PPE on knowing what PPE is necessary: (a) Jobsite, 24 Churchill Road, Pittsburgh, PA: On or about February 18, 2022, employees working aloft in trees and using a chainsaw to cut limbs were not provided training on the requirement to have two means of being secured while cutting with a chainsaw. An employee removed his lanyard prior to making a cut in a Silver Maple tree and accidently cut his climbing line with a chainsaw. He fell approximately 46' from the tree to the ground below, sustaining fatal injuries from the fall.
