29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems identified in 1910.28(b)(1)(i)(A)-1910.28(b)(1)(i)(C). On or about February 18, 2022, with respect to an establishment located at 1101 Perry St, Algoma, WI 54201, employees were exposed to fall hazards of 5 feet and 7.5 feet due to several open areas in the guardrail system around the lower pit of the large Horizontal Boring Machine in the Saw Area.
