29 CFR 1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about and prior to February 22, 2022, the employer failed to develop and document machine specific procedures for the control of hazardous energy that included the intended use of the procedure, specific procedural steps for shut down, isolation and securing machine energy sources, specific procedural steps for the placement of lockout/tagout devices and specific requirements for verification of effectiveness of the procedures for the following equipment in the Machining department/area: a. the Kitamura MyCenter HX800iL (HM-21), b. the OKK MCH 600 (HM-22), c. the OKK MCV 820 (VM-11), d. the YAMA Seiki machining center (VM-12), e. the Haas vertical and horizontal mill (VM-13), and f. the Mitsubishi MVR 30 (FM-51).
